Flight report August        2003
HI 10        PHNL        PHKO        747-400 PS1        FL110        174NM        44 minutes
Engine #1 failure at VR, continued take-off and climb, restarted engine during cruise; smoke air conditioning emergency successfully corrected.
HI 11        PHKO PHNL        747-400 PS1        FL260        32 minutes
During descent, encountered volcanic ash, with flame-out of all engines. Perfomed appropriate maneuver, with 180 turn, quick descent, engine restart and landing on the alternate, in this case PHOG. One extra night in Maui for our passengers!
HI 31        PHOG        PHNL        747-400 PS1        FL120        130NM        35 minutes
Uneventful flight, to bring passengers and repaired plane back to our hub.
HI 61        PHNL        PHTO        747-400 PS1        FL210        226NM        57 minutes
Bleed overheat #1, this means loss of associated engine reverse thrust, so I did not use it on landing, using instead max brakes (no overheat, anyway). I also performed an ILS-noise abatement procedure: this means lowering gear and landing flaps just 500ft above ground, ignoring those loud alert horns and bells.
